Pharmacist
Location: Salt Lake City, Dallas, Denver
Reports to: General Manager or Pharmacy Manager

Summary:
Provides and coordinates pharmaceutical services. Performs pharmaceutical care activities for customers / patients within the service area and monitors, documents and reports patient outcomes, provides or coordinates the dispensing of medications, home medical equipment and ancillary supplies within the customer service site area and participates in on-call schedule
Essential Duties and Responsibilities:
- Responsible for evaluating and monitoring drug therapy relative to the total needs/problems of the home care patient in collaboration with physicians, nurses and other healthcare professionals
- Adheres to organization policies and procedures. Ensures compliance with all applicable state & federal laws and regulates governing pharmacy practice
- Adheres to all appropriate standards of accreditation. Actively participates in quality assurance activities
- Coordinates and facilitates new referrals/admissions through collaboration with nurses, customer service and distribution
- Reviews, receives/verifies all new patient orders and documents the same on the respective order form
- Assists the physician and other healthcare professionals involved in planning and implementing patient care in the home to ensure safe and effective drug therapy
- Conducts and documents initial and ongoing assessment of specific patient, drug and disease information to identify real and potential drug-related problems or needs
- Assists the physician and nursing staff in setting therapeutic objectives for drug therapy and establishing parameters for monitoring the safety and efficacy of drug therapy
- Monitors the outcomes from drug therapy by collecting, evaluating and interpreting the results of laboratory data, clinical data and other pertinent patient information
- Communicates the results of therapeutic drug monitoring activities to the physician, nursing and other appropriate persons
- Provides consultation and answers inquiries from patients, families, physicians and other healthcare professionals regarding drugs and their use
- Maintains thorough and appropriate documentation in the patient/client record of all clinical activities and communication with patients, physicians and other healthcare professionals
- Participates in the preparation of progress reports for the referring physician on a regular, periodic basis
- Processes and checks all medications, supplies and equipment prior to dispensing from the pharmacy
- Maintains pharmacy records in accordance with the applicable laws and regulations
- Maintains clinical skills and knowledge of pharmacotherapy by reviewing the literature and attending in-services, seminars and conferences
- Required to work on-call evenings and weekends on an as scheduled basis
- Performs other tasks or special projects as requested by management
Knowledge and Skill Requirements:
- A minimum of two years recent experience in an acute-care setting or one year recent experience in home infusion therapy
- Experience with compounding infused drugs
- Must have a proven record of evaluating and monitoring drug therapies for home care patients
- Good organizational and time-management skills
- Participation and membership in professional societies and organizations is desirable
- Excellent verbal and written communication skills
Education/Licenses/Certificates required:
- BS in Pharmacy and a Registered Pharmacist License in state employment required. A PharmD is preferred
- A minimum of two years recent experience in an acute-care setting or one year recent experience in home infusion therapy
Physical and Environmental Requirements:
- Minimum travel may be required
- While performing the duties of this job, the employee is regularly required to sit and talk or hear. The employee is frequently required to walk and use hands to finger, handle or feel. The employee is occasionally required to stand and reach with hands and arms. The employees must frequently lift and/or move up to 10 pounds and occasionally lift and or move up to 30 pounds. Specific vision abilities required by this job include close vision, distance vision and the ability to adjust focus
